Prior to our first session, I will send you an invitation to create a Client Portal. This is where we may communicate about your schedule, paperwork, and other issues in a HIPAA secure manner. You may keep the Privacy Form.
Please bring your insurance card with you to your first session.

• Client Therapist Agreement (for all clients)
• Intake Questions (for adult clients)
• Child Intake Questions (for parents to complete for child clients)
